For the dissolution of a New Jersey LLC, the only fee you would have to pay is $100 for an NJ Limited Liability Company and $125 if it is a Foreign Limited Liability Company.
A Form L-109 needs to be completed in order to dissolve the LLC. Also, New Jersey State requires "that all annual report obligations should be satisfied prior to submitting the dissolution paperwork."
Furthermore, checks should be made payable to "TREASURER, STATE OF NEW JERSEY" and mailed to:

PO Box 308,
Trenton, NJ 08646
