Question Employer errored payment to another State

I just need some help trying to figure out what and how to file for a refund to the State of Colorado. Employer paid them by mistake and now I have to get the money back. I went to Colorado Tax website and I don't know if I have to file more than the Form 104PN, since there is no signature line to sign attesting to the truth and accuracy on this form. Do I have to do some other type of paperwork to send to them or not?

I live in Texas and already purchased Turbo Tax Deluxe (without State) since we don't have a state tax. I don't want to purchase the State from Turbo Tax since it isn't my fault, but the employer's fault.

I have my Federal ready to file, but I don't want to file until I know what has to be done concerning the State of Colorado for the refund!

If someone could just tell me what to do.
Help, please!
