What is Form 5329 Additional Taxes on Qualified Plans used for?

The main purpose of Form 5329 "Additional Taxes on Qualified Plans" is used to report additional taxes on the following distributions;

1. IRA's
2. Other Qualified Retirement Plans
3. Modified Endowment Contracts
4. QTP's
5. Archer MSA's
6. HSA's
7. Coverdell ESA's
