Appointee Information Form8821

I already have a Form 8821 on file with the IRS, I need to expand the tax years and add specific information to the appointee authorization. What form should I use?

I guess you need to file a new form 8821.Anewly filed form 8821 automatically revokes any previously filed form 8821.
